At Markiewicz Dental of Mundelein, our goal is to help you keep your teeth for a lifetime. We will address both your short-term and long-term dental needs and work with you to develop a treatment plan that both ensures your dental health and fits within your budget.
Understanding Dental Insurance
Dental insurance is not traditional “insurance” (a payment to cover the cost of a loss) but rather a benefit designed to assist with the cost of routine dental care. Most plans cover only a portion of the total cost of necessary dental treatment. For example, while a dentist may recommend a crown for a tooth with extensive decay, your dental plan may only cover the cost of a filling. This does not mean that a crown is unnecessary—it simply means that your benefit is limited to a filling.

Insurance Information
As a courtesy to our patients with dental insurance, we will submit claims to your insurance company free of charge. To facilitate this process, we require a copy of your insurance card and/or insurance policy at your first visit of each benefit year.
We strive to help you maximize your insurance benefits by submitting pre-authorizations upon request prior to restorative treatment, giving you a better understanding of your coverage. However, given the wide variety of insurance providers and individual plans, it is not possible for us to know the specifics of every plan. It is your responsibility to be aware of your specific insurance coverage and limitations.
If your insurance has not paid within 90 days of services rendered, you will be required to make full payment to Markiewicz Dental. Should your insurance company issue payment after 90 days, we will provide you with a reimbursement check.
In-Network Provider
We are an in-network provider with Delta Dental Premier. If you have Delta Dental Premier, we require payment for your deductible and estimated co-pay on the day of service.
Payment Policy
For patients with dental insurance benefits, claims for preventive services will be submitted to your insurance. After processing, you will receive a statement for any remaining balance, which is due upon receipt.
For non-preventive services, payment is due on the day of treatment unless prior arrangements have been made. If you have Delta Dental Premier, we require payment for the deductible and estimated co-pay on the day of service.
For your convenience, we accept Visa, MasterCard, American Express, and Discover credit cards, as well as EBT cards. We also offer a discount for pre-payment.
Helping Make Treatment Affordable
3% Courtesy Discount
Patients who pay in full with cash or check at the time of service will receive a 3% courtesy.
Internal Payment Plans
With no additional fees or interest, we offer our patients the option to split their treatment costs into two or three monthly payments.
Third-Party Financing
We offer flexible payment plans through Cherry Financing, allowing you to receive the care you need with manageable monthly payments.
Financing applications: CareCredit, Lending Club, Cherry. Ask the front desk for a Cherry application.
Already have a statement?
Preventive balances are billed after your insurance processes the claim and are due on receipt. You can settle them online.
